Contraceptive Pill/Patch/Ring request

We are asking patients to complete a short questionnaire as part of the Review.

You must have an up to date Height, weight and Blood Pressure reading to complete the form.  If you don't have a BP machine at home this service is available for some Chemists and after 11am from Reception at the surgery.

Practice Nurse/Pharmacist will review information provided and update your records. Prescription will be issued or they will contact you if further information required.
